Please find below additional information for returning and new students:
1. Medication – Holy Cross Catholic School requires permission to administer meds via this FORM, it needs to be filled out and signed by a doctor. This includes but is not limited to, all prescription meds, over the counter meds, cough drops, etc.
2. Sports Physical – All students must have a current sports physical to participate on our sports teams. Please have your doctor fill out this form and return to the office. If you already have a sports physical it cannot be dated later than April 15 of the previous school year. Ex: If you are entering the 26/27 school year, the sports physical must have a date of 4/15/26 or later to qualify.
3. Immunizations – The SCCHD collects status from Holy Cross School on where students stand with their immunizations. Please make sure they are up to date or that you have obtained a waiver if opting out. The waiver can be obtained online by going to the St. Clair County Health Dept website for further instructions. AOD requires that you have one or the other.
4. Kindergarten Requirements (if you already provided these to the office, thank you):

Copy of birth certificate

Copy of baptismal certificate

Completed preschool physical

Copy of immunization record

Copy of dental screening record (this is a new requirement from the SCCHD)

results of vision & hearing screenings

5. PK Requirements (if you already provided these to the office, thank you):

Copy of birth certificate

Completed preschool physical

Copy of immunization record

Child Information Record – CLICK HERE – this is required by LARA and your child cannot attend without it.

6. All parent/family/friend volunteers must have completed Protecting Gods Children and have a completed background check (the AOD runs their own background checks, any check you had done at another facility will not count for HC). Both of these will be handled through virtus.org. When choosing your diocese select “Detroit”.

Who does this pertain to? Anyone who wants to help, volunteer, go on a field trip, supervise lunch, help at Santa Shop, help at Field Day, Harvest Dinner, Fish Fry, Bingo, attend a classroom party, anything that involves interaction with our students at a school sanctioned event or volunteer opportunity. PGC has now been extended to include treatment of elderly and those with disabilities. PGC cert. is required for parish dinners and events as well if you are working them.  If you do not have these two items you will not be allowed to chaperone/help at the function.

If you are attending events like PK or 8th grade graduation, the talent show, school Mass, etc and are only a spectator you are not required to have PGC or a background check as you will not be helping with the students or parish members.

7. Service Credit Tracking – Each family is required to track their own service credit hours in FACTS Family Portal (new process starting 26/27 school year). You will be able to add each event worked, as well as input any hours that grandparents/friends and other family members worked on your behalf. You will be able to see what your balance is.
